If you live in the US, localharvest.org seems like a good site to find a local CSA.
Here's a list of CSAs in Canada (not comprehensive and very few have websites).
Here's a list of organic farms and markets in Ontario, including CSAs.
Here's a link to an Ontario CSA Farm Directory. Nasty amount of scrolling and they appear to only list one for Toronto, which is incorrect.
If you live in Toronto, ON, here's a list of CSAs that deliver to the GTA:
March 2008 note: there were a couple more, but one doesn't appear to be online anymore (Nature's Way Organics) and Thurston Organic is discontinuing their Toronto delivery in favour of the local market.

- Coopers Goat and Veggy Farm - as of June 2/08 they still had shares available but only deliver to the 'burbs (Oshawa, Pickering, Ajax, Whitby, Brooklyn, Markham, Richmond Hill, Stouffville, Newmarket, Aurora, Queensville, Keswick, Sutton, Jackson's Point, some areas in Scarborough, and some in North York) but plan to change that next year.
